Confirmation Registration

Dear Parent/Guardian,
I am writing to you today because you have a child who may wish to make his/her Confirmation. I invite you and your child to prepare with us during the coming months for this sacrament. In our Family of Parishes we use a process for preparing your child for this sacrament called "My Weekly Mass Journal; Preparing for Confirmation". The most important thing of all is that you are welcomed and we will give you all the resources needed to really prepare your child well for this turning point in his or her life. On the back of this letter is an outline of the program. Please complete the Registration Form noting there is a $40.00 donation to assist the Family of Parishes to help cover the costs of materials. All donations should be made payable to Most Precious Blood Church by cash, cheque or e-transfer (Most Precious Blood email - mpbwin@dol.ca). Please include with the registration form a copy of your child's Baptismal Certificate unless the Baptism took place at one of our Churches within our Family of Parishes. I have asked my Associate Pastor, Father Innocent lbeh to take the lead on this year's program along with many dedicated and devoted catechists from across the Family of Parishes who will assist him. If you have any questions please call Father Innocent at 519-948-4133 extension 223. All registration forms and registration fees should be dropped off to Most Precious Blood Office (1947 Meldrum) no later than Tuesday, March 28th. Please note: Office Hours are Tuesday-Friday 9 a.m. - 12 noon and 12:30 p.m. - 4:00 p.m.

representatives, successors and assigns, hereby:• Represent that I am/we are the parent(s), legal g uardian(s) or other

authorized person(s) or organization with authority over the Participant. • Consent to the Participant participating in the

Event, and/or possible use of photo and video.

*For those baptized at parishes other churches mentioned a photocopy of their baptismal certificate is required.
